At 'Empowered 2 Support' we offer a personalised support planning and brokerage services tailored to your needs along with a reliable payment service to simplify your experience.
'Empowered 2 Support' is an Individual Service Fund (ISF) provider. This is usually referred to as an ISF by local councils or 'Third-Party Budgets' by health commissioners.
We offer people the choice and control around their support as well as avoiding the need to manage the finances. This is a good option for people who do not want the full flexibility and responsibility of managing a Direct Payment and don't want the funding authority (Social care or health) to directly commission and manage their support.
Think of it as a middle option - the best of both where we manage your personal budget for you.

Support Planning
Our support planning and brokerage service is designed to help individuals create a personalised plan that outlines their care needs, goals, and preferences. We work closely with you to ensure your support plans align with their lifestyle and aspirations and the assessed needs from your social care or health assessment and find local services within your community.

Budget Management
With our budget management service, we assist individuals in effectively managing their allocated funds. We will ensure that the allocated budget is utilised to meet the agreed needs and outcomes on your support plan. A dedicated bank account will be held by us as your Individual Service Fund (ISF) provider. We will pay for the agreed services on your behalf.

Care Coordination
Care coordination is a key aspect of our services, where we facilitate the communication between providers and yourselves. We assist with compliance and contracts, negotiating terms and rates ensuring that you receive the best possible service within your budget. This also includes flexibility when things don't go to plan and potential.
How it works

Assessment / budget setting
You must have a care assessment from your local health service or social care team and be allocated a Personal Budget.
If you are eligible for support tell them you want an Individual Service Fund (ISF) managed by 'Empowered 2 Support'.
You can contact us before, during or after your assessment for more information or guidance.

Support Plan and brokerage
A contract or agreement is arranged between 'Empowered 2 Support' and the commissioning authority (council or health).
We will manage this if you have chosen us to support you.
We will arrange to meet you and discuss what support you need.
When you are happy we will arrange the approvals process and once agreed we will start setting up the services for you.

Financial Management
‘Empowered 2 Support’ will set up the bank accounts and ask for the personal budget to be transferred to us.
We will arrange the contracts for you and pay for the support on your behalf.
We will be responsible for ensuring the personal budget is spent as per your support plan.
